Validation and Registration Information
Registration Information Three documents are usually used to register participants. A registration form, a parent/guardian permission form, and a medical release form. Registration forms vary by association, but generally include the same type of information. An example from the New Fairfield Falcons can be seen by visiting the Falcons' website. If you would like the Word document used by the Falcons as a starting point for your registration form send an email to PastPresident@nffalcons.org

Link to National Pop Warner Birthdate checker Link to National Pop Warner Age Weight Matrix Medical Release Form This is a Microsoft Word file with a medical release form that you may use for your association. Suggestion: Add your association mailing address or other contact information to the bottom of the form. After you download and open the file, click on "view" on the Microsoft command bar at the top of the screen, and then click on "footer and header". Go to the bottom of the form and enter the information into the footer area. You can include mailing address, phone number and web site address for example.

Link to Scholastic Forms on National Pop Warner website It has been decided that we will NOT use the new Scholastic Certification Form that was introduced in February. We will use the 2007 Alternative Scholastic Eligibility Form (the modified old 3B form). The form must be signed by the parent and school prior to the participant taking part in practice like it always has been done. The stipulation with this form is that a participant can play/cheer all during regular season, however they must provide a passing report card or progress report showing satisfactory progress before they can participate in Regional Playoff's/Regional Competition. They would need to provide this before the start of our first Regional playoff game. This would hold true for cheerleaders participating in Regional Competition, even though Competition is not until late November. The Report Card or progress report can be obtained anytime after the first Monday in October. This gives everyone plenty of time to provide the above. If a participant does not provide the satisfactory Report Card or progress report, by the first Regional playoff game, they are dropped from the roster.
